Meet Dr. Daliva
Dr. Daliva's journey in medicine began with a deep connection to the elderly community. From volunteering at the Jewish Home for the Aging to participating in programs like D.A.T.E at the University of California Davis, Dr. Daliva's experiences shaped a unique approach to healthcare. Dr. Daliva makes four promises to the families of their patients: 1) Caring for your loved one as their own. 2) Advocating for the best services. 3) Open and reliable communication. 4) Ensuring quality over quantity. During these challenging times, Dr. Daliva aims to be the reliable and responsible physician your loved one needs.

Dr. Daliva holds a D.O. in Osteopathic Medicine from Western University of Health Sciences, along with an M.S. in Medical Sciences and a B.S. in Neurobiology, Physiology & Behavior and Chicano studies from U.C. Davis. He is also ABIM Board Certified in Internal Medicine and holds multiple licenses and certifications.

Dr. Judy Ly, the founder of Align Medicine, is a passionate physician dedicated to patient advocacy. With a focus on family medicine and a special interest in geriatric and palliative care, Dr. Ly co-founded Align Medicine to guide patients through complex healthcare transitions. Beyond medicine, Dr. Ly enjoys baking, gardening, and spending time with family.

HI! I love working in healthcare and I have a background in Medical Assisting! My passion for patient advocacy comes from personal experience supporting my own mother for years while she was on hospice care, which has inspired me to help create a compassionate and welcoming experience for every patient and their family. I’m grateful to work with an amazing team that I truly enjoy every day. Outside of the office, I am a mother of three and love spending my free time with my husband and kids.
