What is Transitional care?
Transitional Care Management helps patients move safely from the hospital back to everyday life at home. It includes follow-up check-ins, help with medications, and coordination with doctors to make sure recovery stays on track and to prevent return visits to the hospital.
The standard level of care
All patients after hospitalization are instructed to follow up with a personal care provider with no intent to help you or your loved one on the path to recovery once you leave the hospital doors. Even if you decided to live in a nursing facility or at home no one is their to advocate for you along your path to healing.

Afraid you might not know what is going on with your loved one? We maintain direct communication with both the care facilitators and you to keep everyone informed and help aid you in your family members progress of healing.
