We want to save you money and time! Based on our initial discovery session meeting, these services will be recommended to you. You may or may not need the full scope of our services and just one or two of our services to fit your needs.
MEDICAL CONCIERGE SERVICE
A direct line of communication via phone and text with dedicated personnel that will address any health-related concerns or facility concerns and direct them to the appropriate team member.
MEDICAL COORDINATOR SERVICE
Dedicated medical assistant to help arrange appointments with primary care physicians and specialists.
PERSONALIZED PHYSICIAN SECONDARY CARE
one on one conversation with the doctor

Continuity of care by the same doctor from the hospital to different types of facilities ( memory, skilled, board and care) to home with home health.

Concerns with the facility are addressed by our physician and staff within 24 hours.
​
Care is provided only by a board-certified internal medicine doctor, no physician assistant or nurse practitioner.

MEDICAL SECOND OPINION
Our doctor can provide a medical second opinion via phone or video
LIAISON AND ADVOCATE BETWEEN SPECIALISTS, PRIMARY CARE AND FAMILY
Our doctor will communicate with your primary care physician or specialist.
COMMUNICATION VIA PHONE/TEXT/EMAIL
Communication with the team Monday - Friday, 8 AM-5 PM via text, phone, or email. Weekends observed.
Nursing Care
A separate nursing visit assesses any further needs missed by the facility.
